STEP 2: General Family Information
Download the applicable documents, complete and return them to the Admissions Office.

• Directory & Appearance Clause Information – Required
• Grandparent & Others Information
• Parent Driver Information – complete if you feel there may be a time when other Brook Hill students may need to ride with you to a destination (field trip, other campus, sporting event, etc.)
• Brook Hill Parent Association (BHPA) – complete if you wish to become a part of the Brook Hill Parent Association.

STEP 3: Grade Level Information
Click on your child’s enrolling Grade Level to continue
PK – 5th Grade
6th – 12th Grade – Day Students Only
8th – 12th Grade – Boarding Students Only

Once you have downloaded and completed all required forms, you must send them to the Admissions Office.  You can fax, mail or email them to:

• TAPPS Physical Evaluation – required ONLY if the student will be participating in any sports activities (this includes cheerleading).
• Student Passenger/Driver – Required ONLY if the student will be riding with anyone other than a family member OR if the student will be driving themselves.
• 6th-8th Grade Schedule Request form - required
• 9th Grade Schedule Request form - required
• 10th Grade Schedule Request form - required
• 11th Grade Schedule Request form - required
